All patients registered with this surgery are required to have a named, accountable GP. Your named GP will be Dr R Kumar, Dr D Taunk or Dr Kasliwal.
Please note that patients can see the GP of their choice.
Are you age 75 or over?
We are required to ensure that there is a named, accountable GP assigned to each patient aged 75 and over. This will be the GP that you have been registered under (not necessarily the GP you usually see).
The named accountable GP will take lead responsibility for ensuring that all appropriate services required under the contract are delivered to each of their patients aged 75 and over where required (based on the clinical judgement of the named accountable GP).
They will:
- work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient
- ensure that the physical and psychological needs of the patient are recognised and responded to by the relevant Clinician in the practice
- ensure the patient aged 75 years and over has access to a health check
This does not prevent you from seeing any Clinician in the practice as you currently do.
