Can you register as a patient with us?
If you live within our catchment area we can register you as a patient however, you will need to provide photographic evidence and proof of your address, if you can. You will then be asked to complete a ‘GMS1 Registration Form’ and a patient questionnaire. After registration, you will be given an appointment for a ‘General Health Check’ with our Practice Nurse. The Health Check is for patients 5 years and over. We also have an extended catchment area. This includes some postcodes beginning with WS2.
Please note that all children being registered under 18 years will need to fill in an additional form stating names of parents & school attended - No child under 18 years will be able to join the practice list without their maternal parent being registered at the same time (i.e. mother –exceptional circumstances considered)
If you live outside of our catchment area you may be able to register with the practice- as an “out of area” patient but would not be entitled to home visits. The GP will assess each out of area application and will decide whether the application is clinically appropriate. We do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance or disability when registering patients.
